Form W-9
Use the IRS’s official Form W-9 (Rev. March 2024) to give Veris CPA your taxpayer identification number and certification. Download it, complete and sign it, then return it to your Veris contact.
A completed W-9 contains your taxpayer identification number, so please do not send it by ordinary email. Ask your Veris contact for the best way to deliver it securely.
